Campership Program – Camp fee assistance program. The deadline (Wednesday, March 15, 2017) is set to ensure communication to recipients before the early bird deadline occurs.
Parental/Guardian Authorization Form – Every Scout attending Treasure Valley MUST have a signed Parental Authorization form. This form authorizes certain adults to transport your scout from camp. Please include the scout master and an authorized adult in your list. Also included is the Shooting Release form for scouts wanting to get a shooting (archery, rifle, or shotgun) merit badge. Over the Counter Medication Release form (OTC Release Form). Please do not mail this form please make a copy and bring the copy to camp the day that you check in. Required by the state of Massachusetts, do not print double sided
BSA Annual Health and Medical Form: Parts A, B & C required. Special Note on Camp Medical Form
All Youth AND Adults attending BS Resident Camp at Treasure Valley are REQUIRED to present a completed Medical Form upon arrival. Participants who do not have the appropriate signatures will not be permitted to stay in camp. Please note that BSA form must be filled out, attached doctors forms are okay with them.
